Change My Contact Details?

If you change your name, address or telephone number, please let us know. If you move outside the practice area it may be necessary for you to find a new doctor in your new area. In exceptional circumstances, you may be able to stay as a patient of the practice whilst you are receiving treatment for a specific condition. However, once treatment has been completed you will be required to re-register with another practice. This is to ensure that the practice can meet the demands of patients within its practice area.

The local Primary Care Trust will help if you experience any difficulty.
